Board-Certified Physicians
Physicians at Gifted Hearts Hospice and Palliative Care are always close by to ensure that each patient receives the best treatment options and monitoring. The hospice physican will assume care as primary physician and will be the lead clinician in formulating an individualized plan of care for each patient. At Gifted Hearts Hospice and Palliative Care, the physican will be available to consult with patients at their residence to provide prompt care if required, thus limiting the need for patients to leave home for medical attention.
Nursing Services

End of life circumstances are difficult enough to cope with. The essence of palliative care is to minimize symptoms and create a comfortable environment. Gifted Hearts Hospice and Palliative Care has a compassionate team of nurses that you can count on each step of the way. They are available 24/7 for all your medical needs. The nurse is usually first on site to assess patients for admission and through follow up visits. The nurse works in close contact with the physicians and other personnel to ensure that optimal care is provided.
Certified Nursing Assistants
We generate a calm and relaxing environment where patients are monitored consistently to ensure that they are comfortable. Gifted Hearts Hospice and Palliative Care aides are qualified care providers. Our team is here to assist with personal care issues including bathing, changing of clothing and bedding, grooming and oral hygiene. Care is customized to each patient's needs.
